Briefly describe what aspects of chronic care, self management, health literacy, consumer perspective or quality improvement interest you?: for our people to be independant in the care being offered to be comfortable to ask questions, appropriate supports offered, and feel their needs are being met by our Practice. this includes obesity, diabetes,respiratory, cardiovascular, health literacy needs improvements by our practice.
