Briefly describe what aspects of chronic care, self management, health literacy, consumer perspective or quality improvement interest you?
I work for an organisation supporting Doctors practices to provide services for the long term conditions their patients have. We also have a role in Public Health promotion in the region supporting programmes that have beneficial impacts which aim to reduce the onset of long term conditions or improve quality of life for those with long term conditions.
