I am keen to get an idea what type of care plans are being used around New Zealand to support self management

Hi all,

In Auckland there are several groups looking to update our paper based Care Plans used for Care Plus.
Has anyone else around the country come up with a good format? What has worked or not worked in terms of encouraging patients/clients to bring them everytime and clinicians to write in them regularly?
I do think they can be a useful tool (when used well) to help everyone in the person's care team to be on the same page so to speak.
Look forward to hearing other people's experiences with care plans.
i am a great fan of wellness recovery action planning which was primarily designed for mental health self management. however, the principles of this can be used by anyone for anything. it is simple to use and allows you to easily manage your life. i am the nz train the trainers for wrap so if you want further information please feel free to contact me.
I agree and believe there's a lot we can learn from mental health around WRAPS,s strengths approach and building resilience. Great to know you are the NZ trainer. Are there any resources, ideas or examples you can share via the resource library? Thanks, Janine
